Reference ranges are time-of-day specific and assay-dependent. A random cortisol without a documented draw time is difficult to interpret. Diagnosis of excess or deficiency relies on dynamic tests, not a single value.

Why timing dominates the number

Cortisol has one of the strongest daily rhythms of any hormone: it surges in the early morning to help you wake and get going, then falls across the day to a nighttime low. A level of, say, 15 µg/dL is expected in the morning but abnormally high at midnight. This is why clinicians care as much when the sample was drawn as what the value is — and why interpreting a random cortisol without a timestamp is unreliable.

Too much and too little — the real conditions

Cushing's syndrome is genuine cortisol excess, usually from a tumor or from long-term steroid medication; its diagnosis rests on specific tests such as late-night salivary cortisol, 24-hour urinary free cortisol, or an overnight dexamethasone-suppression test — deliberately not a single blood draw. Adrenal insufficiency is genuine deficiency (Addison's disease or pituitary causes), diagnosed with a morning cortisol plus an ACTH-stimulation test. Both are important to catch, and both need the right confirmatory testing rather than a lone number.

"Adrenal fatigue" vs. adrenal disease

The popular idea of "adrenal fatigue" — that everyday stress exhausts the adrenal glands and causes chronically low cortisol and tiredness — is not supported by good evidence, and a systematic review found no consistent link between cortisol measures and the fatigue symptoms attributed to it. Real adrenal insufficiency is a specific, testable diagnosis with clear consequences. If you're persistently exhausted, that deserves a proper evaluation — but the answer is rarely a random salivary cortisol panel, and treating a non-existent "fatigue" with cortisol supplements can be harmful.

What time should a cortisol blood test be drawn?

The standard is early morning, around 8 a.m., when cortisol is naturally at its peak. Because the hormone follows a daily rhythm, the draw time is essential context — a value that's normal in the morning could be abnormal at night. Always note (and ask your lab to record) the time of the draw.

Is "adrenal fatigue" a real diagnosis?

No — it isn't recognized as a medical diagnosis, and systematic reviews haven't found a consistent relationship between cortisol levels and the tiredness symptoms attributed to it. Genuine adrenal insufficiency is real and testable; persistent fatigue is worth investigating, but usually for other causes.

My cortisol was high on one test — do I have Cushing's syndrome?

Not necessarily. Cortisol rises with acute stress, illness, and even the blood draw itself, and it's naturally high in the morning. Cushing's syndrome is confirmed with specific tests — late-night salivary cortisol, 24-hour urinary free cortisol, or a dexamethasone-suppression test — not a single elevated value.

Can I test cortisol with saliva at home?

Late-night salivary cortisol is a legitimate, validated test used specifically to screen for cortisol excess, because the nighttime low is where abnormalities show up. General multi-sample "cortisol curve" panels marketed for stress or fatigue, however, aren't a reliable way to diagnose adrenal disease. Interpretation should be done with a clinician.
