HDL cholesterol ("good cholesterol") is inversely associated with cardiovascular risk — low HDL is a reliable *marker* of risk, especially as part of the insulin-resistance pattern. But it's not a lever you fix in isolation: trials that raised HDL pharmacologically didn't reduce events, and very high HDL is paradoxically associated with higher mortality. HDL is best read as a signal within the whole lipid and metabolic picture, not a target to maximize.

Sex-specific cutoffs matter: the low-HDL threshold is < 40 mg/dL for men and < 50 mg/dL for women. HDL is one input to overall risk, not a standalone verdict.

Low HDL as a marker vs. a target

Decades of epidemiology show low HDL tracks with higher cardiovascular risk, and it's built into risk calculators for that reason. The catch: that's an association. When trials raised HDL directly — niacin added to statin therapy in AIM-HIGH, and extended-release niacin plus laropiprant in HPS2-THRIVE — cardiovascular events did not fall, and side effects rose. The lesson: low HDL is a useful warning light, but pushing the number up pharmacologically doesn't fix the underlying risk. Address the cause (usually insulin resistance and lifestyle) rather than the number.

The high-HDL surprise

"Higher is always better" turns out to be wrong at the extremes. In large Danish general-population cohorts, people with very high HDL (roughly > 100 mg/dL in men, > 135 in women) had higher all-cause mortality — a U-shaped relationship. The mechanism isn't fully understood, but it means an unusually high HDL is a reason to look at the whole picture with a clinician, not to celebrate.

What to read alongside HDL

HDL is most informative in context:
- Triglycerides — high triglycerides + low HDL is the classic insulin-resistance dyslipidemia
- Triglyceride / HDL ratio — a practical metabolic-risk surrogate; < 2.0 is favorable
- LDL-C and ApoB — the causal, treatable side of lipid risk (ApoB counts atherogenic particles)
- Lp(a) — an independent, largely genetic risk marker checked at least once
- Fasting insulin / HbA1c — the metabolic drivers behind a low HDL
A "normal" HDL alongside high triglycerides and high ApoB is not reassuring; the pattern matters more than any single value.

Can I raise my HDL naturally?

Regular aerobic exercise, quitting smoking, and reducing insulin resistance (through diet, activity, and weight loss) can raise HDL modestly. But the goal is improving the underlying metabolic health those changes reflect — not the HDL number itself, which pharmacological trials showed isn't independently protective when forced up.

My HDL is high — is that good or bad?

Moderately high HDL (60–80 mg/dL) is generally considered favorable. Very high HDL (well above ~80–100) has been linked to higher mortality in large cohorts, so an unusually high value is worth reviewing with your doctor alongside the rest of your labs rather than assuming it's protective.

Why is my HDL low even though I'm healthy?

HDL has a genetic component and is also lowered by high triglycerides, insulin resistance, smoking, and some medications. A low HDL with otherwise excellent metabolic markers (low triglycerides, normal insulin, low ApoB) carries different weight than low HDL within a full insulin-resistance pattern.

Does fasting matter for HDL?

HDL itself is fairly stable fed or fasted, but it's usually drawn as part of a lipid panel that includes triglycerides — and triglycerides do require a 9–12 hour fast for accurate interpretation. Follow the fasting instruction given for the panel.
