Reference ranges and antibody cutoffs vary by laboratory and assay. The 2010 ACR/EULAR classification criteria score 'low-positive' and 'high-positive' serology differently — a titer more than 3× the upper limit carries more weight than a borderline result.

Frequently asked questions
Does a positive rheumatoid factor mean I have rheumatoid arthritis?
No. RF appears in 5-10% of healthy people (more with age), in chronic infections like hepatitis C, and in other autoimmune diseases. Without joint symptoms, a low-titer RF is usually an incidental finding. Anti-CCP is far more specific — and even it is interpreted alongside the clinical picture, not instead of it.
What's the difference between RF and anti-CCP?
Both are autoantibodies found in RA at similar rates (~70%), but anti-CCP is much more specific — around 95% — so a positive result is far stronger evidence for RA. High anti-CCP titers also predict more erosive disease, which influences how aggressively treatment starts.
Can I have RA with completely normal blood work?
Yes. About 25% of RA patients are seronegative (negative RF and anti-CCP), and CRP/ESR can be normal in early or well-controlled disease. RA is ultimately a clinical diagnosis built on persistent synovitis — labs support it, they don't gatekeep it.
My anti-CCP is positive but I feel fine. Will I definitely get RA?
Not definitely, but the risk is real: anti-CCP antibodies can precede symptoms by years, and high titers in a person with joint symptoms strongly predict progression. This finding deserves a rheumatology consult and a monitoring plan rather than either panic or dismissal.
Why do I need blood tests every few weeks on methotrexate?
Methotrexate is highly effective and generally safe — under surveillance. CBC catches bone-marrow suppression, liver enzymes catch hepatotoxicity, and creatinine matters because the drug clears through the kidneys. Typical schedule: every 2-4 weeks at the start, stretching to every 8-12 weeks once stable.
Do normal CRP and ESR mean my RA is in remission?
They're necessary but not sufficient. Composite scores (DAS28, CDAI) combine the labs with joint counts and your own assessment — and some joint damage progresses even with quiet markers. Conversely, CRP can be elevated for reasons unrelated to RA. The markers are a dial, not a verdict.
Why does my rheumatologist care about my cholesterol?
RA roughly doubles cardiovascular risk — chronic inflammation accelerates atherosclerosis independently of traditional risk factors. Controlling disease activity lowers that risk, and standard prevention (lipids, blood pressure, smoking cessation) matters more in RA, not less.
Can RA go into remission and can I stop my medications?
Sustained remission is a realistic goal on modern treat-to-target therapy. Careful tapering (reducing dose or spacing) is sometimes possible after 6+ months of stable remission — but abrupt stopping carries a high flare risk, and flares aren't always fully recaptured. Taper only with your rheumatologist, with labs and joint checks along the way.
