Download Your Dermatology Records From a ModMed Portal
By Phi Longevity
Sign in to your dermatology practice's patient portal on a computer. In the ModMed portals we checked, results are under Tests and Results, and each one has a View Result PDF option that you can save or print. For visit notes, pathology reports or anything not listed, ask the practice's records office for an electronic copy. That is a right you have under HIPAA, and our explainer on that right covers the timing and fees.
If you also see doctors at a hospital, see how to download lab results from MyChart or from Oracle Health.
Way 1: Save each result as a PDF
- Sign in to the practice's patient portal on a computer.
- Open Tests and Results, or the section with a similar name. Menus vary by practice.
- Open a result and choose View Result PDF, then save or print the file.
- Save each file with the test name and the date in the file name, so you can find it later.
Way 2: Ask for a copy of your record
- Call the practice or send a portal message and ask for the medical records office. HealthIT.gov (ONC) says you can also look under Contact Us on the practice's website.
- Ask for an electronic copy of your record, and say which visits, notes or pathology reports you need. HHS says you can ask for the form and format you want, such as a PDF, if the practice can readily produce it.
- Write down the date you asked. HHS says the 30-day response clock starts on the day the practice receives your request.
See or stop the apps that read your records
In the ModMed portal we checked, a page called SynApps, under your profile icon, lists every app you have allowed to read your records and has a Revoke Access button next to each. ModMed's developer documentation says that once you sign in to a patient app with your portal login, a link appears in your portal so you can use the app again or turn off its access.
What to do with the files
Put your dermatology files in one place with your other records. Phi Longevity PRISM reads PDFs, photos of printouts, spreadsheets and C-CDA files, combines them with records from your other doctors (you can connect MyChart or Oracle Health accounts directly), and builds a one-page Care Team Brief that shows each result, its date and how it has changed. A direct connection to ModMed practices is not available yet, so for now upload the PDFs you saved. Once your records are in, you can also connect your own Claude or ChatGPT to read your summary.

What Phi Longevity PRISM does not do
It does not diagnose, and it does not replace your dermatologist. It reads lab results today; medicines, diagnoses and allergies are not imported yet, and dermatology records often carry few lab values. If a result is not in your files, it cannot show it, and if it cannot read a file it tells you instead of guessing.
Frequently asked questions
How do I know if my dermatologist uses ModMed?
Look at the portal's web address or login page, or ask the front desk which patient portal and records system the practice uses. Many ModMed practice portals have an address that ends in modmedapp.com, but some practices use their own address.
Can I download all my dermatology records at once?
In the ModMed portals we checked, results open one at a time as PDFs. For a complete record, including visit notes and pathology reports, ask the practice's medical records office for an electronic copy. HHS says you can ask for the format you want, such as a PDF, if the practice can readily produce it.
What is SynApps in my ModMed portal?
In the ModMed portal we checked, a SynApps page under the profile icon lists the apps you have allowed to read your records, with a Revoke Access button next to each one. ModMed's developer documentation says patients can manage or disable an app's access from inside their portal.
