Should I take B vitamins to lower a high homocysteine?
If the cause is a genuine B12 or folate deficiency, correcting it is worthwhile for the deficiency itself. But taking B vitamins purely to push down the homocysteine number, in the hope of preventing heart attacks or strokes, isn't well supported — large trials lowered the number without clearly lowering those events. Discuss the reason behind your level with your clinician rather than treating the number alone.
Does high homocysteine cause dementia?
High homocysteine is associated with faster cognitive decline in observational studies, but association isn't proof of cause, and B-vitamin trials have shown mixed and generally modest cognitive effects. It's reasonable to correct a true deficiency; it's not established that lowering homocysteine prevents dementia.
Do I need to fast for a homocysteine test?
Many labs prefer a fasting sample because a recent high-protein meal can transiently raise homocysteine. Follow your lab's instructions, and try to use the same lab and conditions if you're tracking it over time.
My homocysteine is mildly high but my B12 is normal — what now?
Mild elevations can reflect borderline folate or B6 status, reduced kidney function, or simply lab variation. It's a prompt to review kidney function and vitamin status with your clinician, not a diagnosis on its own.
