Total B12 is an imperfect test with a wide indeterminate range. Symptoms plus MMA/homocysteine often matter more than the total number, which can be falsely normal (or falsely low in pregnancy and with some medications).

Why the total-B12 number can mislead

Standard tests measure total B12, but only a fraction is actually available to cells.
