How to Connect Your Medical Records to Claude

November 2, 2026·6 min read

By Phi Longevity

There are two ways to give Claude your medical records today. One links your patient portals directly. The other brings records from every doctor, including paper and PDF records, into one place first and lets Claude read a summary. This page explains both, using each company's own pages. It is general information, not advice about your situation.

Route 1: The HealthEx connector

  • Who can use it. Claude Academy says it is available to Claude Pro, Max and Ultra users in the United States at no additional charge.
  • How it connects. You verify your identity through CLEAR (Claude Academy lists a phone number, email, selfie and a photo of a US government ID), then log in to each health portal. You can add as many providers as you want, or skip one.
  • Suits you when every doctor you see is on a patient portal you can log in to.

Route 2: Phi Longevity PRISM as a custom connector

  1. Bring your records into Phi Longevity PRISM at philongevity.com: connect MyChart (Epic) or Oracle Health at participating health systems, or upload PDFs, photos of printouts and portal downloads from any doctor. Records from every doctor become one record you control.
  2. Sign in and open Connect Claude or ChatGPT. It gives you the address to paste.
  3. In Claude, go to Customize, then Connectors, click "+ Add", then "Add custom connector", and paste the address. Anthropic's help center lists these steps and says custom connectors are available on Free, Pro, Max, Ultra, Team and Enterprise plans (Free allows one; on Team and Enterprise plans only owners can add them).
  4. Sign in with the emailed link and choose what to allow on the permission page.
  5. Ask Claude to prepare you for your next appointment, explain what changed, or check which tests may be due.

What the assistant gets: a read-only summary of which results are high, low or normal, how they are changing, and what to ask your doctor. Exact lab numbers too, unless you tick “hide my exact lab numbers”. If you allow the Health Picture, it also sees Phi Longevity's whole-record summary and your own medicine list. Never your name, birth date, address, email or payment details, and never your uploaded files or doctor's notes themselves. Two extra permissions are optional: adding documents to your record, which wait for you to accept them in the app, and noting what your clinician said, which you confirm in the app. You can end the connection any time on your Connected AI assistants page, and a connection lasts 90 days at most.

  • Suits you when some of your records are on paper, in PDFs, or from doctors who are not on a portal, or when you want a one-page Care Team Brief for your doctors as well.

Before you connect anything

  • Anthropic's help center says custom connectors reach services Anthropic has not verified, and to connect only servers you trust. Only you can set up the connection to your Phi Longevity PRISM account: Claude cannot read anything until you sign in and tick the permission page.
  • HHS says a provider generally cannot refuse to send your records to an app you chose, and that the HIPAA Rules do not restrict how that app uses them. Read each service's privacy terms.
  • For what leaves your hands when you use an assistant with records, see using your MyChart records with ChatGPT or Claude. For what is live for AI assistants at Phi Longevity, see our page on MCP and assistants.

What to do with the files

If you have records from several health systems, our guide to combining medical records and downloading lab results from MyChart cover how to collect them. If you choose Phi Longevity PRISM, this is how we handle your records:

Phi Longevity is not a healthcare provider or a "covered entity" under HIPAA for your personal account. We store your records in HIPAA-eligible systems under a signed business associate agreement with Google Cloud, and we protect them to that standard. You can ask us to delete your data at any time.

What Phi Longevity PRISM does not do

It does not diagnose, and it does not replace your doctor. It does not send your records to an assistant unless you set up the connection yourself, and it does not tell you which assistant to use.

Frequently asked questions

Is there a Claude connector for my medical records?

Yes. Claude Academy describes the HealthEx connector, available to Claude Pro, Max and Ultra users in the United States at no additional charge, which links your patient portals after a CLEAR identity check. Phi Longevity PRISM can also be added to your own Claude as a custom connector: you bring in records from every doctor at philongevity.com, by connecting MyChart or Oracle Health or uploading files, and Claude reads a summary with your permission.

Can Claude read records from a doctor who is not on a patient portal?

Through Phi Longevity PRISM, yes, if you have the records as files. You upload PDFs, photos of printouts or portal downloads from any doctor at philongevity.com, and they become part of the same record your Claude can read a summary of. Claude Academy's HealthEx tutorial describes logging in to each health portal and does not mention uploading files.

Which Claude plans can add a custom connector?

Anthropic's help center says custom connectors using remote MCP are available on Free, Pro, Max, Ultra, Team and Enterprise plans, that Free users are limited to one custom connector, and that on Team and Enterprise plans only owners can add them.

Can I stop Claude from seeing my records later?

Yes. With Phi Longevity PRISM you can end the connection any time on your Connected AI assistants page, and a connection lasts 90 days at most. You can also remove the connector inside Claude.
